Stretta Procedure 43257

Does anyone know if Noviatus Medicare carrier for PA cover the stretta procedure, CPT® 43257.

The majority of the insurance carriers we deal with currently will not pay for this, but our medicare carrier does not have a policy or article indicating coverage.
The NY carrier we deal with NGS does not pay for this.

Just wondering if anyone was familar wtih Medicare coverage.

Here is the LCD- the procedrue code is listed as covered but it depends on the indication. Look at section B on page 3
