Let's see the answers we get to this question:
PCP sees patient in office and does allergy testing (95004). Sends results to another physician to do preparation and provision (95165). The PCP then gives injection (95115). Question--does the physician who does preparation and provision have to examine the patient to bill code 95165.? Resources, if any.

no- the fee is for preparation- radiologists rarely meet their patients...

but the allergist should- there is a heck of a liability in providing a service to a patient without an established relationship.
