Does anyone have any successful sample letters appealing (requesting re-determinations) Professional RAC appeals? specifically appealing denials for .. "new patient E& M codes are payable only if no face to face services have occured within 3 years"....

Have you received RAC denial letters? I have not heard of any RAC requests to physicians for E&M services. I need details! What state, what specialty, what did the letter say?
