I would love to have some example notes for when it is ok to bill a 99211. I have gotten some off from AAFP but I keep getting push back on the clarity of billing a 99211 with a b-12 as opposed to why not with a BP check. I push back that it must be medically necessary etc etc. Example notes would be helpful if anyone can provide or direct me to a site.

A B 12 shot requires the nurse skill to administer but no thought process. Room patient, give shot, pt goes home full of energy and vitality.
A BP check can involve thought process- BP high, doc says increase med. BP too low, cut back dose. Documentation is the key to an appeal. If it is "BP 120/80" then no 99211, if it is "BP 150/100 doc says double lisinopril" then 99211 should pass audit

Intradermal TB administration. Physician orders, patient comes back to have it administered, 99211, then have it read later at no extra charge.

Both CPT and CMS have said that reading the PPD may be billed as a nurse visit.
