What codes can I use when a PCP does an eye exam by using the Snellen Chart as part of the office visit?

99173 Screening test of visual acuity, quantitative, bilateral - (The screening test used must employ graduated visual acuity stimuli that allow a quantitative estimate of visual acuity [eg, Snellen chart]. Other identifiable services unrelated to this screening test provided at the same time may be reported separately [eg, preventive medicine services]. When acuity is measured as part of a general ophthalmological service or of an E/M service of the eye, it is a diagnostic examination and not a screening test.). CPT 99173, is intended to be used primarily in the pediatric age group (birth through age 17) and is a screening test of visual acuity only.
Note: Most Insurers consider codes 99172 and 99173 to be included in (and not separately reimbursed from) Preventive Medicine services, problem-oriented E/M services, and ophthalmology E/M services. Check each payers policy.
Warning- personal commentary
Wonder why there is a health care crisis and loss of confidence in the medical profession? Perhaps because some doctors try to charge for such simple things like a Snellen acuity test. What is next? An extra charge for each vital sign taken?

Yes, the AMA added editorial comment in 2009 to support clinicians in billing for this with well visits. It is true that it is a small service, comparatively, but some groups do it a lot of times, and the reimbursement can add up, if you can get your insurers to pay it.
