ROS in e/m coding

Does anyone count the past medical or social history part of a note towards the ROS? We have been told that, for example, if it is noted in a patient's PM&SH that he has diabetes, that this can be counted for one point under endocrine in the ROS. I thought that the ROS was for the physicians actually asking the patient questions regarding any problems or signs/symptoms in the body systems.
