All services must be medically necessary.   signaturedoc Nancy's right Posted: Mar 10 2010, 4:47 PM I should add that you never want to use the diagnosis code that applies to the surgery (OA hip, cholecystitis, colon cancer, etc) - that is sure to generate a denial as the surgeon is getting paid a global fee for that diagnosis.
