IV CPT® code 90765 gets denials each time we bill

I always get denial from insurance company for CPT® 2008 code for IV 90765. we and the payer both use CPT® and icd9 2008.
For instance, a patient present with gastritis and the doctor treats the patient with Omeprazole through IV and assign CPT® code 90765 with icd9 code for gastritis 535.50 and we get denial from insurance company mentioning in Denial Description that Service is not clinically indicated based on good clinical practice without additional supporting diagnoses/activities or sometime they mention Service is not clinically indicated based on good clinical practice.

Why in the world are you giving it IV? I totally agree with insurer- there is no reason in the office to give IV Omeprazole.
