Posting Charges

When posting multiple CPT's and DX codes, can we use the same dx codes for each cpt charge entered, as long as the primary dx remains the same? For example on the super bill the Doctor checks off CPT 99214, 93000, 90732, and 36415, and includes DX 786.09, 784.0, and 790.6. Does each cpt need a particular dx that is related to the cpt for example for cpt 36415 use dx 790.6 as a primary because it is related to blood? Or would I use the same dx for each cpt with one main primary dx?

Diagnosis codes

Every service provided requires medical necessity and that means a diagnosis code linked to each service code, as appropriate.

In other words....

in each case the diagnosis code must make sense for the test/procedure. 790.6 (abnormal chemistry) does not really justify an EKG; 401.1 or 428.1 or 786.50 do make sense. 790.6 makes sense for an office visit but not for a mole removal or stool hemoccult test.
