When we design paper encounter forms, (or select the frequently used codes for the favorites in an electronic charging service) we face two conflicting demands.  On the one hand, we want to have as many codes as possible on the form, to give the practitioners as many options as possible.  On the other hand, we only want to include what they use most often, so the printing doesn't become too small to read!

When deciding what E/M codes to include on the encounter form, include all the codes in any category.  That is, if the form includes outpatient consults, put all five levels of consult on the form.  Don't be tempted to leave off the level 1 "because we hardly ever use it, and they can write it in if they need it."  The OIG considers that you are implying to the physician that they should only use higher level codes.  Include the entire range of codes in any one category.
